	General Price List Northern Star Mortuary, Inc. These prices are effective as of January 13th, 2010 but are subject to change without notice. The goods and services shown within this General Price List (GPL) are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. We realize that this is a highly-detailed list of services and prices and may be overhelming to review on your own. Please feel free to call us at (913) 400-3766 with any questions you may have. BASIC SERVICES OF FUNERAL DIRECTOR & STAFF $1,850.00 This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charge for direct cremation, immediate burials and forwarding or receiving remains). Our basic service charge includes consultation/ arrangement conference with our client families or responsible party, preparation and filing of necessary consents, authorizations, permits, certificates, newspaper notices, staff assisting in the arrangements and funeral prior to and following the services, coordinating with those providing other portions of the funeral such as the cemetery, crematory or other third parties, assisting with forms and notary services. EMBALMING $600.00 EXCEPT IN CERTAIN SPEICAL CASES, embalming is not required by law. Embalming may be necessary, however if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. OTHER PREPARATION OF THE BODY $230.00 This includes washing, restoration, dressing, cosmetizing and casketing your loved one. There will be an additional charge for autopsy cases of $150.00 TRANSFER OF REMAINS $280.00 This includes the initial call, regardless of the time of death. This service entails picking your loved one up from the residence, hospital, nursing home and transporting to the mortuary. (Within a 25 miles radius). Additional charges of $2.50 per loaded mile will be added when applicable. Note If a first call is made by our firm and you decide to transfer your loved one to another "Funeral Home"you are obligated to pay our firm the services rendered, such as the first call transfer and embalming if given permission. USE OF STAFF AND EQUIPMENT Our facilities are not equipped to accommodate funeral services, however we can handle the services at the home church or at the church we contract as the use of our chapel. If this is not feasible for the families and you choose another location to hold the services you are responsible for that payment. Funeral Ceremony $380.00 Visitation/Viewing $320.00 Memorial Services $300.00 Visitation/Funeral Ceremony, same day $550.00 Graveside Service $295.00 TRANSPORTATION & AUTOMOTIVE EQUIPMENT ALL CLIENT FAMILIES HAVE 24 HOURS BEFORE THE SERVICE TO NOTIFY THE FUNERAL HOME OF A CANCELLATION OR THE ORDER OF ANY FUNERAL SERVICE VEHICLE. Hearse $400.00 (Within a 25 mile radius). Additional charges of $2.50 per loaded mile will be added when applicable. Limousines $375.00 The limousines seat seven family members, eight with the driver. Families have the option of providing their own transportation. When they do so, they will be directed to meet at the services at a certain time. They will not be escorted by the director. (Within a 25 mile radius). Additional charges of $2.50 per loaded mile will be added when applicable. If you require the limousine to wait during the repast there is a $70.00 charge per limousine, families have a maximum of one hour. NOTE: The funeral vehicles are on a schedule to return to the funeral home at a specific time. If you surpass your one hour, the funeral assistant will have to leave and the families will be responsible for obtaining a ride back to their destinations or you may pay another $70.00 for another hour on the spot and you will be issued a receipt. In most areas of the country, state or local law does not require that you buy a container to surround the casket in the grave. However many cemeteries require that you have such a container so that the grave will not sink in. Either a grave liner or a burial vault will satisfy these requirements. This does not include additional charges for autopsy, merchandise such as the casket, shipping container, airfare (shipping charges), visitation rites or ceremonies prior to forwarding of the body, cash advance items such as the sales tax and death certificates. RECEIVING OF REMAINS FROM ANOTHER MORTUARY $1,830.00 This charge includes basic services of funeral director and staff, local transportation to the mortuary and or place of local disposition. This does not include visitation, rites of ceremonies, cash advanced items flowers and programs. IMMEDIATE BURIAL $1,250.00-$3,221.00 Our charge for immediate burial includes basic services of funeral director and staff, transfer of remains to the mortuary, dressing, casketing and transportation to the cemetery. This excludes rites or ceremonies, cemetery charges, outer burial container, or graveside services.This charge does not include embalming which is not required for immediate burials. This excludes visitation, ceremony rites and the cremation fee. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition material (with or without an outside covering). DIRECT CREMATION WITH ALTERNATIVE CONTAINER $1,617.00 DIRECT CREMATION WITH CONTAINER PROVIDED BY PURCHASER $1,250.00 ALTERNATIVE CONTAINERS $367.00 An alternative container is used when you select to arrange a direct cremation. Alternative containers encase your loved one and can be made of fiberboard or other composition materials. CASH ADVANCED ITEMS Sales Tax (is only added on to the merchandise that you select e.g. casket, programs, urns, burial clothing, and outer burial container). Certified copies of death certificates: Kansas death certificates are $15.00 for the first certified copy and $15.00 each additional. Missouri death certificates are $13.00 for the first certified and $10.00 each additional. Cemetery (opening, closing & plot): Crematory: Airline/Railroad: Markers/Headstone: Flowers: Musician/Organist: Clergy: Newspaper Obituaries: Note: The Kansas City Star allows up to nine lines of basic information for free, this includes the Name of your loved one, age, date of death, place of death (optional) date, place and time of the visitation and funeral ceremony and interment and arranging funeral home. If the newspaper information exceeds more than the basic information your funeral arranger will contact the obituary department at the Star for additional charges. Kansas City Call: The charge for the Call paper is $50.00 for the photo and obituary and $40.00 for the obituary. Kansas State Globe: The charge for the Kansas State Globe is 35.00 for the photo and the full obituary. IN ALL INSTANCES, CASH ADVANCED ITEMS MUST BE PAID IN FULL AT THE TIME OF THE ARRANGEMENTS Funeral Service/Cremation Packages The packages listed below are other options our families can choose from. For your convenience we accept cash, personal checks (which are verified before the arrangement conference is concluded), cashier’s checks and all major credit cards. All assignable Life Insurace polices. The Funeral Home will assist the family or responsible party with filing all Life Insurance Policies. To do so, the original policies or policy numbers are needed along with a certified copy of death certificate. Policies over two years old are accepted, however policies under two years old cannot be accepted. If you anticipate any difficulty in meeting these obligations, please feel free to discuss it with us. MERCHANDISE WARRANTIES** Northern Star Mortuary, Inc. makes no representation or warranties about the product value of caskets, outer burial (vault) containers, or urns other than those made by the manufacturer. The only warranties, expressed or implied, granted in connections with goods sold with service contracts are the express written warranties, if any extended by the manufacturer thereof. The seller extends no other warranties or merchandising fitness of a particular product.
